Add library name, correct pipeline tag, link to code and copy README content

This PR corrects the pipeline_tag to image-text-to-text which better reflects the model's capabilities. It also adds the library_name as transformers and links the code to the model card. This PR also copies the content from the Github README into the model card for better presentation.
